General Information
Description of ServiceNurse-Family Partnership (NFP) is a free, voluntary prevention program that provides nurse home visitation services to low income, first-time mothers prior to their 28th week of pregnancy. Nurses begin home visits early in the mother’s pregnancy and continue visitation until the child’s second birthday. Nurses provide support, education and counseling on health, behavioral and self-sufficiency issues. First-time mothers can expect to receive referrals to healthcare, childcare, job training and other support services available in their community. The programs’ goal is to help women have a healthy pregnancy and a healthy baby, become a great parent and build a strong network of support for herself and her family.

Intake ProcedureReferral can be completed by Health Care Navigation Specialist or Appointment Setter at 2-1-1 San Diego. See Eligibility for criteria.

EligibilityAll referrals must be low Low-income.

Low-income first time pregnant mothers (including teens) and/or any woman with a high risk pregnancy (i.e. no prenatal care after 1st trimester, history of multiple pregnancy loss, substance abuse during pregnancy, gestational diabetes, hypertension, current domestic violence.)

OR

Low-income first time mother postpartum with a history of or current postpartum depression, current domestic violence or bonding issues.

OR

Low-income first time mother with a high risk infant (i.e. birth weight less than 1500 grams or premature, developmental delays, perinatal substance abuse exposure.)
