General Information
Description of ServiceProvides public health nurses to promote and improve access to healthcare and other resources for MCH, PHN and their children. With a focus on maternal and child health issues and on communicable disease control. Services may be provided in the home or in a clinic. Public health nurses are BSN-registered nurses with course work in community health.

EligibilityCHILDREN 0-5 YEARS:
High-risk infants with low birth weight, Hypotonia, Prematurity of ≤32 wks, APGAR scores of 0-3, Intracranial abnormal pathology, genetic, systemic or metabolic conditions which may be associated with developmental delay, Perinatal substance abuse exposure, positive toxicity screen for infant or signs of toxicity/withdrawal.
Service Duration: 18 month of services to infant/child.

For CWS Referral Only: Children < 6 y with medical condition; first time entry into CWS and within 3 mos of involvement; must be with parents or in kinship care.

PREGNANT/POSTPARTUM WOMEN:
Any pregnancies with current/recent domestic violence, history of prenatal loss or pre-term births, no prenatal care after 1st trimester, high risk pregnancies,
Postpartum families with current/recent DV, postpartum depression/bonding issues.

Types of FeesFixed fee schedule. Minimal fees charged for services. Some services available on a sliding fee scale basis.

Payment NotesAccepts Medi-Cal for some services. No one will be turned away based on inability to pay.
