Description of Service:
Provides assistance and information and referral services to connect uninsured children and pregnant women with no-cost or low-cost health and dental insurance, including expanded Medi-Cal, Healthy Families, and other low cost healthcare programs. Provides initial eligibility screening and referrals to the most appropriate health insurance or medical care options. Answers questions about available healthcare programs and sends application forms to clients within 2 to 5 days). Clients who are experiencing difficulty in completing application forms can obtain assistance over the phone or in person. Follow-up is provided to ensure families connect with resources.
