General Information
Description of Service:
AIM provides low cost health insurance coverage to uninsured, middle-income pregnant women. The total cost is 1.5% of the subscriber's adjusted annual household income. The State of California and the Federal Government supplement the subscriber contribution to cover the full cost of care.

An application can be printed from the AIM website at www.aim.ca.gov and mailed to: Mail Application to: The AIM Program P.O. Box 15559 Sacramento, CA 95852-0559 or by calling 1-800-433-2611.
Note: A completed application must be received by the end of the 30th the week of pregnancy.

Copies of: • Pregnancy certification; • Income verification documents; • Proof of income deductions; and • A $50 cashier’s check or money order.
Note: Your complete application must be received by the AIM Program prior to the end of your 30th week of pregnancy in order to be considered for the AIM Program. If you are near your 30th week of pregnancy, you may send your application overnight via Fed-Ex, US Postal Service, etc to: AIM 625 Coolidge Drive, Suite 100 Folsom, CA 95630

Intended Participants:
Middle income families who do not have health insurance (including Medicare Part A and Part B benefits as of the application date) and whose income is too high to qualify for no-cost Medi-Cal. Uninsured, middle income pregnant women.
AIM is available to those who have insurance if their deductible or co-payment for maternity services is more than $500.00.

Eligibility:
Pregnant, but not more than 30 weeks pregnant, as of the application date.
California resident.
Not a recipient of no-cost Medi-Cal or Medicare Part A and Part B benefits as of the application date.
Uninsured or covered by private insurance with a separate maternity deductible or co-payment of more than $500.00 Monthly household income within the AIM Income Guidelines.

The total cost for your coverage is 1.5% of your annual household income.
