Description of Service:
CMS is a health care program for indigent adults ages 21 to 64. It covers certain medically necessary services.

Intake Procedure:
Call for eligibility screening information. If program eligibility requirements are met, patients receive an identification card. Patients can apply for coverage for a single emergency room visit by completing an emergency room application at a contracting hospital. Some community health centers offer applications for short-term (20 days) eligibility for urgent primary care services.
